Luigi Mangione: Personal Injury Turned Into Murder Case
Luigi Mangione, the accused UnitedHealthcare CEO killer, had been living with a spinal condition known as spondylolisthesis, which results from a fractured vertebra slipping out of place. The condition can cause debilitating pain if the misaligned bone compresses nearby nerves, and can significantly impact daily life.
The 26-year-old Ivy League graduate had been living in Hawaii for some time when, after a six-month stay at a co-living space, his back pain worsened, partly due to a surfing accident. Increasing pain and his inability to participate in certain activities led the Maryland native to consider back surgery. Mangione began writing about his decision on a Reddit post published in July 2023.
“I got caught in this loop for a year, all the while putting my life on hold in my 20’s and damaging my nerves while I waffled on the decision. I have surgery scheduled in two weeks and I keep wondering why I was so afraid of it.”

He continued to document his recovery on his social media platforms. He posted a picture of what appeared to be an X-Ray of his back to his X account. In it, a rod and some screws attached to a spine were visible. His recovery seemed successful as he continued to post about it. Mangione even said, “it was way less of a big deal than I had anticipated”. He mentioned being surprised not to be in pain only days after the procedure.
The Role of Health Insurance in the Luigi Mangione Case
Health insurance can significantly reduce costs for patients, especially in complex cases that require extended care or surgery. Some common surgeries can exceed the $100,000 mark. However, even those who have health insurance can face significant financial challenges. These usually come in the form of high co-pays, out-of-network costs, or denied claims, among others.
It is unclear at this point whether Luigi Mangione had to deal with any of these hurdles. Additionally, there is no evidence he was ever insured by UnitedHealthcare.
Even so, Luigi expressed discontent with the health industry, with comments alluding to medical professionals disregarding concerns from patients. “Tell them you are “unable to work” / do your job. We live in a capitalist society. I’ve found that the medical industry responds to these key words far more urgently than you describing unbearable pain and how it’s impacting your quality of life” he wrote on a Reddit comment.
From Ivy League Graduate to Murder Suspect
So, what drove Luigi Mangione to kill UnitedHealthcare CEO, Brian Thompson?
After his arrest, a manifesto was discovered where he expressed his anger towards the health insurance industry. A transcript by independent journalist Ken Klippenstein reads, “Frankly, these parasites simply had it coming. A reminder: the US has the #1 most expensive healthcare system in the world, yet we rank roughly #42 in life expectancy.”
Although Mangione was not insured by UnitedHealthcare, this insurance company is currently the largest in the United States by market share.

- November 18, 2024: After a period of decreased social media activity, Luigi Mangione’s mother filed a missing person’s report in San Francisco. This report was filed 16 days before the shooting.
- November 24, 2024: Mangione arrived in New York. According to his arrest warrant, the ID he used to check into his Upper West Side hostel was fake.
- December 4, 2024: Roughly an hour after leaving his hostel, surveillance footage captures Luigi Mangione coming up to Brian Thompson outside the Hilton—where his company’s annual investor conference was taking place—and shooting him in the back. He is thought to have left New York that day.
- December 9: An employee at a McDonald’s in Pennsylvania tips off police. Mangione is taken into custody and presents police officers with the fake ID that had been used at the NYC hostel. He was held without bail and declined to enter a plea in his first court appearance. He appeared at an extradition hearing the following day.
What Charges is Luigi Mangione Facing?
- New York Charges: 11 counts, including three counts of murder and a terrorism offense, which carries a maximum sentence of life in prison.
- Federal Charges: Two counts of stalking, one count of murder through use of a firearm, and one count of firearms offense. This could bring the death penalty if he is convicted.
- Pennsylvania Charges: Five counts, including possession of an unlicensed firearm, forgery, and providing false identification to police.
Pretrial Court Hearing for Luigi Mangione
Mangione, charged with murder and terrorism, among other charges, appeared in a New York state court on February 21 for his first pretrial hearing. If convicted of using a firearm in the murder, he can face the death penalty. However, it is not known at this moment if prosecutors plan on seeking the sentence.
At the hearing, Mangione pleaded not guilty to state charges. He has yet to enter a plea for his federal charges. The federal charges carry a potential maximum sentence of life in prison without the possibility of parole.
